Location & Contact

mapThe Placenta Clinic is part of the Fetal Medicine Unit of Mount Sinai Hospital, and it uses the resources of the Centre for Excellence in Obstetrical Ultrasound (CEOU). Both of these are located on the 3rd floor of the Ontario Power Generation (OPG) building at the intersection of College St. and University Ave. directly above the Queen’s Park subway station (click here for directions and parking).

How to make an appointment

To make an appointment, please print and complete the Special Pregnancy Program referral form for your Placenta Clinic consultation and fax it to (416) 586-3216. The referral request will be assessed by the fetal medicine triage nurse.
